I hereby certify that the information contained on this Employment Application Form is true and correct to the best of my knowledge and agree to have any of the statements checked by RTTEMPS, LLC (hereinafter referred to as “the Company”), unless I indicate to the contrary. I understand that any misrepresentation, falsification or material omission of information on this Application may result in my failure to receive an offer or, if I am hired, my dismissal of employment. I authorize the references listed above as well as other individuals, to provide the Company with any and all information concerning my previous employment and any other pertinent information that they may have. Further, I authorize RTEMPS LLC to contact my former employers and references, to examine records of employment, education records, licensure/certification checks and any other information pertinent to my employment and I release, hold harmless and indemnify all parties and persons from any and all liability for any damages that my result from the attainment and /or furnishing of such information as well as from the disclosure of such information by the Company or any of its agents, employees or representatives.
I further understand that by completing this Application, the Company is NOT under any obligation to interview me for a position, nor is this Application to be construed as an offer of employment. I understand that this Employment Application is valid for thirty (30) days from this date and if I wish to be considered for employment after thirty (30) days, I must update and advise the company in writing of any changes in the information provided in the application. I agree to furnish any additional information which the Company may request as may be required to complete my Employment Application.
I further understand that all offers of employment are conditioned upon the Company’s receipt of satisfactory responses to all medical information requests, criminal and sex offender background checks, consumer reports, proof of legal authority to work in the U.S., and any and all alcohol/drug testing required by the Company. If all such information is satisfactory, any offer of employment is conditioned upon the Company and RTTEMPS LLC’s client’s acceptance of me as a candidate for the position.
In the event that I am offered and accept employment, I understand such employment is “at-will”, unless otherwise agreed to by the Company in writing.
